We need to collect personal information so we can provide services you at the practice.

Collected personal information will include your names, addresses and contact details, medical information including medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors.

Your personal information will be held confidentially at the practice in electronic records. This clinic holds all personal information securely in protected information systems.Personal information will only be used for the purpose of providing medical services and payments, unless otherwise consented to. We will inform you where we have a statutory requirement to disclose certain personal information (for example, some diseases require mandatory notification).

We will not disclose your personal information to any third party without full disclosure to you beforehand, and will include the reason for the information transfer and seek full consent from you.

Exceptions to disclose without your consent are where the information is required by law; necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent; to assist in locating a missing person; to establish, exercise or defend an equitable claim; for the purpose of a confidential dispute resolution process.

We will not use any personal information in relation to direct marketing to you without your express consent. You may opt-out of direct marketing at any time by notifying the practice in a letter or email. Appointment confirmation notifications are not considered marketing communication and are handled separately.
